Covid-19 Pre-Treatment Questionnaire

THIS PAGE CONTAINS IMPORTANT INFORMATION.  PLEASE READ BEFORE ATTENDING YOUR APPOINTMENT.

 

1. Do you currently have covid-19 or any symptons of covid-19?

(This might include: a high temperature or fever, a new. persistent or continuous cough,  difficulty breathing, chest pain, loss or change to your sense of smell or taste, headaches, aches and pains, severe lethargy or fatigue, rashes, sore throat, runny nose, congestion.  Please visit the NHS website for an up-to-date list of symptoms.)

If yes, please contact me to re-arrange your treatment.

2. Does anyone in your household or support bubble have covid-19 or symptoms of covid-19?

If yes, please contact me to re-arrange your treatment.

 

3. Have you been in close contact with anyone else in the past 14 days who has symptoms of  covid-19 or been contacted by the NHS Test and Trace service and told to self-isolate?

If yes, please contact me to re-arrange your treatment.

4. Have you returned from traveling abroad within the last 14 days?

If yes, please contact me to re-arrange your treatment.

5. Have you had covid-19?

If yes, please seek permission from your doctor or consultant before your treatment.

6. Are you classed as an extremely vulnerable person (high risk)?

If yes, please contact me to postpone your treatment until the Government indicates it's safe to book in.

7. Are you classed as a vulnerable person (moderate risk)?

If yes, please seek permission from your doctor or consultant before your treatment.

8. Are you currently shielding?

If yes, please contact me to postpone your treatment until the Government indicates it's safe to book in.

 

If you have answered yes to questions 1, 2, 3, 4, 6 or 8 please DO NOT attend your appointment.  You will not be charged cancellation fees under these circumstances and the full pre-paid amount will be transferred towards your next appointment.  Please note you will be required to wait 14 days before re-booking.

Please be aware, I am legally obliged to notify NHS track and trace to provide your contact details if any members of my household or clients report symptoms or test positive for covid-19.
